Replacement Form Instructions

Use this form to request an exchange or after-sales support for devices delivered to your practice. High-priority cases include DOA (dead on arrival) and adhesion failure within 3 days. We usually reply within 1–2 business days.

Prepare before you submit:

  • Order/Inquiry ID and (if available) Shipment ID
  • Device serial/lot, application date, species (dog/cat)
  • Short description of the issue (what happened, when)
  • Photos or short video of the device/packaging/placement site (optional but helpful)
  • Service scope: US/Canada clinics; replacement is subject to verification and availability.
